 Circles Transitions  

 Circles of support projects are working with young people making the challenging transition from school to adult life in the community.

It's tough being a teenager in a fast moving world. Imagine having to cope with al these challenges as well as a learning disability!

Young people with learning disabilities face huge additional challenges every day.

The project works to provide volunteer based support for young people in need.

How does it work?

Using existing contacts, where they exist, and carefully matched volunteers our Mentors will work to build a Circle of Support for the young person.

 millrace image    The Circle meets once a month and plans community based activities which will offer opportunities for personal progression and development. During their time with the project young people are supported by their Circle through Person Centred Planning in identifying their aspirations and aims for the future and with the support of their Circle will begin to put the plan into action.

Most Clients will stay with the project for 2 years with tapering support as necessary on an individual basis .
